I understand that participation may include photographs and video/ audio recordings. Sessions will be age-appropriate, respectful and strengths-based, conducted in a safe environment, and supervised by appropriate adults. I understand there is no payment for participation.

Images and videos may be used for TKMCF website, social media platforms, fundraising campaigns, printed promotional materials (flyers, posters, brochures), event presentations, annual reports or impact reports, or media publications (e.g. newspapers, online features).

By signing this form, I confirm that: - Participation is voluntary and will not affect my child’s therapy services at Kids Matters Occupational Therapy. - I may withdraw consent at any time by notifying TKMCF in writing. - I understand that while TKMCF will remove content upon withdrawal where possible, previously published materials (e.g. printed brochures or shared media) may not be fully retractable. - TKMCF will use all images respectfully and in alignment with its mission to support children and families. - Images will not be sold to third parties.
