Referral form for support
The Kids Matters Community Foundation seeks to support the children of low income families by providing guidance, parent education and funding for specialist supports including assessments, parent consultations and the Circle of Security Parenting Program.
To be eligible for funding, applicants must meet the below criteria:
1. A child or adolescent in need of relevant specialist assessments to enable them to access essential paediatric therapy due to a disability and/or a diagnosed chronic medical condition.
2. The parent, guardian or primary caregiver/s of the person described in (1) are financially disadvantaged/ in the low-income category and do not have access to government funding to meet the needs of their child.
EVIDENCE OF FINANCIAL HARDSHIP
We require all applicants to provide documentary evidence to support their application. Such evidence could be a copy of a recent Centrelink Income and Asset Statement or Health Care Card.
REFERENCES TO SUPPORT APPLICATION
Each application requires two (2) independent written references to confirm their current circumstances and highlights the benefits the proposed financial assistance would provide. Eligible referees can include: GP or other health professional, teachers or early childhood educators or other community support services, Indigenous organisation, public or community housing organisation or similar.

Please note: As a charity, and due to the current economic climate, funds are limited and not all applicants will be successful.
Applications will be presented to the Foundation board (once all documentation is received) and considered on a monthly basis. Applicants will be notified of the outcome as soon as possible.
